The MHSAA Health Questionnaire will not be in use next school year (2021-22).  An MHSAA pre-participation sports physical is again required for all students who participate in sports in 2021-22. A valid 2021-22 physical examination must be completed on or after April 15, 2021.  Please inform students as early as 5th grade of this requirement and date.

Physicals MUST be turned in to your coach at your first practice or anytime to the high school or middle school offices. Students will not be allowed to practice or tryout until a valid physical is on file.

Sports Physicals: For the 2021-22 school year, an MHSAA pre-participation sports physical is required for all students who participate in sports.
